Access Services is seeking a Life sharing Coordinator to connect individuals with IDD to Life sharing provider homes and to support person-centered planning. This hybrid role balances community-based work with administrative responsibilities and requires travel within the Delaware Valley region, primarily from the Fort Washington, PA area.
The coordinator will oversee plans, train providers, monitor homes for safety and regulatory compliance, and coordinate with SCOs, AEs, and community partners
