* Humana is looking for a Care Management Support Professional to support members in overcoming barriers to improved health and well-being in a telephonic environment. You will provide administrative and non-clinical support for the assessment and evaluation of member needs, facilitating appropriate care management interventions and services.
As part of the care management team, the
** Care Management Support Professional
** contributes to the coordination and administration of member support activities in partnership with nurse care managers, social workers, and other interdisciplinary team members. This role is essential to helping members access needed resources and support their journey toward achieving or maintaining optimal wellness.

* Responsibilities Include:
*
* + Complete telephonic, non-clinical assessments with members to identify Social Determinants of Health (SDOH) needs. These needs include concerns related to transportation, food insecurity, housing, utilities, gaps in care, and other potential barriers to health and wellness.
+ Gather relevant member information in a professional and respectful manner, including member needs, concerns, current challenges, available supports, and potential barriers to accessing care or services.
+ Use knowledge of SDOH and community resources to identify, research, document, and coordinate appropriate services that support member health, safety, and overall well-being.
+ Provide education and care coordination to connect members with resources to meet their SDOH needs and close gaps in care.
+ Champion the member experience by guiding members and their families toward appropriate resources in a professional, empathetic, culturally sensitive, and member-centered manner.
+ You will document all interactions with members, providers, community resources, and members of the interdisciplinary care team within the electronic medical record.

* ** Required Qualifications*
* + Bachelor's degree in Social Work, Human Services, Public Health, Psychology, Sociology, Health Administration, or a related field.
+ 2 years' experience working with members, patients, clients, or community populations in a healthcare, human services, social services, public health, or customer support setting.
+ Knowledge of Social Determinants of Health, community resources, and barriers that may impact access to care.
+ Effective written and verbal communication skills, including the ability to conduct telephonic assessments and engage members with empathy, professionalism, and respect.
+ Ability to manage multiple responsibilities efficiently, including answering inbound calls, initiating outbound calls through web-based systems, and navigating multiple platforms with accuracy and ease.
+ 2 years' experience with Microsoft Office applications.
** Preferred Qualifications*
* + Non-clinical case management experience supporting members who have co-occurring medical and behavioral health conditions and financial needs.
+ Familiarity with Medicare and Medicaid
+ Bilingual in Spanish or Creole preferred.

** Hours for this position are Monday - Friday 9:00am-5:30pm EST.

To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:
At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Travel:
While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

* The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$45,400 - $61,300 per year
