WRAPAROUND FACILITATOR:
OnPoint is seeking an energetic, organized, and committed professional to fill the position of full-time Wraparound Facilitator. As a Wraparound Facilitator, you will play a critical role in coordinating team-based interventions for children and youth with severe emotional disturbance (SED) who are involved with multiple community-based services. This position requires a compassionate individual with strong organizational skills to facilitate care coordination, ensure comprehensive treatment plans, and advocate for the needs of children and families.
You will work collaboratively with a multidisciplinary team to develop individualized, strengths-based care plans and support the ongoing needs of clients in a variety of settings. Salary:starting at $57,948.00 up to $79,955.20 annually - placement above minimum salary is based on experience.

- Bachelor’s degree in social work, psychology, human services, child and family studies or related field required.
- One year experience working with children, youth, and families required.
- One year experience with person and family centered service planning and delivery required.
- One year of experience working with children diagnosed with SED and their families required.
- Valid Michigan Driver’s License required.
- State certification as a wraparound facilitator preferred.
- Lived experiences with mental illness/developmental disabilities/ co-occurring disorders/substance use disorders valued.
- Provide support and community-based service that aligns treatment providers and supports to meet the needs of youth and families.
- Engage families, youth, and system partners in a team-driven, strengths-based, and creative problem-solving process.
- Coordinate appropriate referrals to other youth-oriented programs such as outpatient, home based or peer services.
- Adhere to Wraparound fidelity model, as directed through initial and ongoing training.
- Coordinate and facilitate Family Team meetings.
- Develop a comprehensive Wraparound Plan which incorporates the needs of all family members in all domains.
- Document services according to funding, regulatory, and best practice standards
- Performs other duties as assigned.
- Efficient in Microsoft Word and Excel.
- Exceptional written and verbal communication skills.
