Wage Range: $94,857 - $116,113 annually

Posted wage ranges represent the entire range from minimum to maximum. This position is based at Evergreen Health’s Canyon Park Clinic, sharing a space with our primary and urgent care settings. The ideal candidate has experience with providing therapy to adults and is well versed in healthcare. This position works closely on a behavioral health team of four, consisting of four therapists, one psychiatric provider, and a patient service representative who assists with scheduling and phone calls.

Applicants should have experience working on an interdisciplinary team and feel comfortable staffing cases with the primary care and psychiatric provider as applicable. The behavioral health team works cohesively in a shared space, and the position does have a hybrid work from home model (approximately 1-2 days a week would be remote). Access to internet and a secure, confidential space at home without interruptions is required.

Clinic clients are Evergreen Health patients, ages 13+, primarily with diagnoses of anxiety and depression. All clients are referred through their primary care provider at Canyon Park and are anticipated to see either the therapist and/or the psychiatric provider. Therapists are able to see clients for approximately 12 weeks, with the hopes of graduating from therapy or referring out for ongoing therapy options.

Familiarity with evidenced based therapeutic practices is required. Candidate reports to both clinic manager and behavioral health manager.

Job Summary

The Primary Care Therapist position specializes in assessment and treatment of individuals with mental, emotional, or substance abuse conditions. Responsibilities include individual and group therapy, crisis intervention, case management, client advocacy, prevention, and education.

Primary Duties
  • Collaborates with the Primary Care team to coordinate referrals for patients in need of behavioral health therapy.
  • Assess the patient’s psychosocial service needs and create plan in collaboration with patient and the interdisciplinary team
  • Provides crisis intervention, mental health assessment and services as referred or self-referred
  • Provides individual counseling/therapy to patients with mild to moderate mental health symptoms.
  • Manages and maintains appropriate patient panel including referring patients to psychiatry and/or other relevant services (e., Perinatal Mood Disorders, Trauma specialists, Chemical Dependency Treatment etc.)
  • Refers and advocates for clients to access community resources and services.
  • Reinforces care plans and identifies and treats Behavioral Health conditions impeding implementation of medical treatment plans.
  • Facilitates patient treatment groups focused on psychoeducation and behavioral interventions.
  • Ensures proper documentation and needed billing procedures are completed as directly associated with provider’s interventions and assessments.
  • License, Certification, Education or Experience

    REQUIRED for the position:

    • Master’s degree in Social Work, Psychology, Psychiatry, or other equivalent training.
    • Active license as Independent Clinical Social Worker (LICSW), Marriage and Family Therapist (MFT), Mental Health Counselor (LMHC) PsyD or other equivalent Mental Health Licensure in the State of Washington.
    • Demonstrated knowledgeable about behavioral health, chemical dependency and developmental stages.
    • Excellent communication skills in order to effectively communicate ensure, and provide for patient needs, and to provide a legal record of this care.
    • Accountability for professional practice, commitment to patient care, and commitment to uphold the Patient Bill of Rights.

    DESIRED for the position:

    • 2-3 years of experience providing clinical therapy
    • 1 year experience in a healthcare agency/environment
