Cityblock Health is seeking a Care Coordinator to support members with complex medical and behavioral health needs in Raleigh and surrounding communities. You will partner with RN Care Managers to manage caseloads, develop care plans, and address social determinants of health.
You will conduct assessments (PHQ-9, GAD-7, AUDIT, DAST-10), refer to behavioral health programs, and help members access appointments, labs, and community resources while meeting in homes or community settings as needed.
