Cityblock Health seeks a field-based Care Coordinator to support members with complex medical and behavioral health needs. You will manage a caseload, address social barriers, and develop individualized care plans in partnership with members and RN Care Managers.
You will conduct behavioral health screenings (PHQ-9, GAD-7, AUDIT, DAST-10), refer to programs when needed, and help close quality gaps through appointments, tests, and community referrals.
