POSITION SUMMARYThe PROUD (Parents Recovering from Opioid Use Disorder) Clinician provides clinical and case management services for adults ages 18+ in an integrated health care setting that provides primary care, behavioral health and dental services. The Clinician will foster positive working relationships with co-workers, clients and external key stakeholders. The Clinician provides leadership, motivation and support to staff, manages the referrals, admission and discharge processes for clients, and ensures the integrity of the program model.
The PROUD program supports women who are pregnant or parenting young children, and who have active substance use concerns or a substance use history (including opioid use or related disorder).
EDUCATION AND EXPERIENCE/QUALIFICATIONSMaster’s degree in Marriage and Family Therapy, Counseling, Social Work or related license eligible field or a doctoral degree in Psychology is required. Applicant must have knowledge/experience of substance use and working with pregnant and parenting women.
Must hold one of the following credentials: LMSW, LCSW, LMFTA, LMFT, LPCA, LPC or licensed or license-eligible Psychologist in the State of Connecticut.

Works as part of a multidisciplinary team to efficiently and effectively meet the behavioral, primary, and dental health needs of the client, including, but not limited to: providing behavioral health consultation to the primary care provider, assisting client in meeting medical treatment compliance, and referring client to other Wheeler services and/or other community services as needed.
Conducts program specific screening and evaluation, and formulates diagnosis in accordance with the Diagnostic Statistical Manual (DSM). Develops and documents, clear clinical formulations and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant, using behavioral health screening as directed, and are inclusive of behavioral health, primary care, and dental goals as indicated.
Demonstrates knowledge of psychopathology and other behavioral health concerns and primary modalities of psychotherapeutic intervention. Triages clinical emergencies and implements program specific safety and consultation protocols. Utilizes best practices and evidenced based practice models where appropriate and adheres to integrated health model and program expectations.
Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for identified service needs in a timely manner.
