United Home Health Care is seeking a compassionate Physical Therapist to provide one-on-one care in patients' homes. You will conduct initial evaluations and develop personalized plans of care focused on mobility and safety.
Collaborate with physicians, occupational therapists, and nursing staff to support full recovery. Maintain accurate EMR documentation (OASIS a major plus) and teach home exercise programs to patients and families.
