Location: Northern
United Healthcare is seeking a Community Health Worker to support members with complex medical or behavioral needs in the community. The CHW will engage members, develop personalized care plans, and overcome barriers to care, including social determinants of health.
This role requires collaboration with the interdisciplinary care team and a commitment to health equity. The position is field-based with a home-based office, Monday–Friday, with up to 10% travel within Florida counties.
