Location: Northern
Harmonyhomehealth in Albuquerque, NM seeks a licensed Physical Therapist for home health. You will assess patients and develop individualized plans of care to maximize functional outcomes.
In this role you will provide direct home-based interventions, conduct home safety evaluations, and educate patients and families while documenting in the EMR and collaborating with the care team. Must hold a PT license and CPR/BLS, have a valid driver’s license and own vehicle.
