Empowering Families for Success (EFS) is a non-billable program funded by the Department of Mental Health (DMH) Child, Youth and Family Division to provide in-home outreach clinical services. The EFS Program works with families and youth ages 0-22 with acute mental health needs approved for DMH flexible support services. Clinicians will provide in-home and community-based clinical interventions that support the youth and family in achieving specific treatment goals that lead to improved functioning in identified areas of their life.

All clinical interventions are designed to prevent out-of-home placement through enhancing emotional well-being, self-regulation, symptom management, social skills, family resiliency, and self-efficacy in response to challenges posed by severe emotional disturbance, peers, school, and associated stressors on the family system. Clinical services are coherent and integrated with other services and supports in the life of a DMH involved Youth.

This position is part-time, 20 hours per week.

• Establish positive working relationships with youth and families referred to the program.
• Carry a case load working with youth and families who will be seen on a weekly basis for in-home family therapy
• Utilize diagnostic skills involving practical knowledge of the DSM-5 and family assessment tools.
• Will identify and assist the youth and family to obtain and monitor the delivery of available services including medical, educational, and social.
• Provide strengths-based services utilizing a wraparound model.
• Co-create a Community Service Plan with the youth and family and develop treatment approaches that reflect and target the needs identified in the comprehensive assessment.
• Participate in team approach intervention with members of the Flexible Support Services team offering safety planning and treatment services within a therapeutic relationship to assist youth/family in achieving a higher level of functioning.
• Coordinate services to assure that they are integrated and clinically coherent across all treatment providers and supports to the youth.
• Facilitate psycho-educational and therapeutic groups that will focus on social and emotional skill building, community integration, and educational and vocational preparation.
• Maintain a flexible schedule allowing for individualized hours including evenings and weekends if requested
• Be a member of the 24 hour on call telephone support team on a rotating basis
• Maintain record keeping requirements in accordance with agency standards and contractual obligations.
• Maintain ongoing collaboration with other service providers including the Department of Mental Health, on behalf of the youth and family.
• Participate in weekly supervision with independently licensed program director
• Participate in multi-disciplinary team meetings.
• Participate in appropriate clinical and administrative staff meetings.
• Participate in independent education and training opportunities.
• Travel within program catchment area that may include towns/cities between Swansea and Wareham

Master’s Degree in Social Work, Psychology, or a related field from an accredited educational institution required. Interest in child/adolescent/family and intensive home-based interventions. At least 1 year of experience working directly with youth who have experienced serious emotional disturbance or other mental and emotional challenges to well-being.
