Care Coordinator

We are seeking a Care Coordinator to support children with developmental, physical, or emotional disabilities through the Children's Long Term Support (CLTS) Waiver Program.

Serve as the primary contact for families, assisting in accessing services that promote safety and well-being. Responsibilities include conducting assessments, developing and authorizing individualized service plans, coordinating services, facilitating family team meetings, maintaining accurate documentation, collaborating with service teams, and participating in staff training.

• Perform functional assessments and reassessments
• Create and implement family-centered service plans
• Coordinate and authorize services per plans
• Lead family team meetings and maintain records
• Collaborate with multidisciplinary teams and provide legal testimony if needed
• Promote client independence and trauma-informed care using evidence-based practices
• Ensure accurate and timely documentation

• Bachelor's degree in a health or human services field
• At least 1 year of experience working with children in similar populations
• Strong communication, organizational, and documentation skills
• Proficiency with electronic health records and computer applications
• Valid driver's license, reliable transportation, and good driving record
• Bilingual Spanish skills are preferred but not required

The role involves community-based work with daily travel, occasional overnight stays, and responding to crisis situations.
