About the Position

Allied Travel Careers  is working with Towne U.S. LLC to find a qualified Physical Therapist (PT) in Baytown, Texas, 77521!

We are seeking an experienced and compassionate Travel Physical Therapist (PT) to provide home health therapy services throughout the East Region, including Baytown and surrounding areas. The PT will provide individualized care in patients' homes, helping them improve mobility, strength, balance, endurance, and overall functional independence.

This assignment is ideal for a PT with recent home health experience who is looking to take on a travel opportunity. Previous travel experience is not required as long as the candidate has the required home health experience.

Assignment Details
  • Schedule: Monday–Friday, 8:00 AM–5:00 PM
  • Hours: 40 hours/week
  • Length: 8–13 weeks
  • Region: East – Baytown and surrounding areas
  • Setting: Home Health
  • On-Call: None
  • Weekends/Holidays: No weekends or holidays unless approved in advance
  • Start of Cares: No
  • Charting: Well Sky
  • Mileage: GSA maximum
  • Point System: 1 point per visit
  • Weekly Productivity: Approximately 32–34 points
  • Drive Time: Paid and considered when scheduling/productivity is calculated
  • Overtime: Paid when drive time causes the total workweek to exceed 40 hours
Responsibilities
  • Perform comprehensive physical therapy evaluations in patients' homes
  • Develop and implement individualized treatment plans based on patient needs and goals
  • Provide therapeutic exercises, gait training, balance training, strengthening, transfer training, and functional mobility interventions
  • Monitor patient progress and modify treatment plans as appropriate
  • Educate patients and caregivers on home exercise programs, mobility techniques, fall prevention, and safety strategies
  • Provide timely and accurate documentation using Well Sky
  • Communicate with physicians, nurses, caregivers, and other members of the interdisciplinary healthcare team
  • Call in all missed visits as required
  • Communicate regarding evaluation frequency and situations where a patient cannot be contacted
  • Manage a scheduled caseload while meeting weekly productivity expectations
  • Provide patient-centered care while working independently in a home-based environment
Requirements
  • Required: Graduate of an accredited Physical Therapy program
  • Required: Active Physical Therapist (PT) license in the state of assignment
  • Required: Minimum 2 years of recent licensed PT experience
  • Required: Previous Home Health/Homecare experience
  • Private duty experience does not qualify toward the required experience
  • No new graduates
  • Previous travel experience is NOT required
  • Required: BLS certification
  • Required: First Aid certification
  • Strong clinical reasoning, communication, documentation, and organizational skills
  • Ability to work independently and travel between patient homes
  • Reliable transportation and ability to manage a home health caseload

  • Clean, professional attire with closed-toe shoes
  • No facial jewelry except a small nose stud and earrings
  • Candidates should research housing options before submission
