Social Worker, Patient​/Health Advocate

Addus Family Company is seeking a compassionate Social Worker LMSW to support elderly, chronically ill, and disabled clients in home and community settings. Responsibilities

• Conduct comprehensive psychosocial assessments for home-based clients and families
• Develop and update individualized, person-centered care plans
• Coordinate services with nurses, physicians, payers, and community resources
• Provide brief counseling, crisis intervention, and caregiver support
• Advocate for client rights, safety, and ability to remain at home
• Maintain timely, accurate, and compliant documentation
• Monitor progress and adjust care plans based on client needs
• Educate clients and families on resources, benefits, and care options
• Collaborate with interdisciplinary teams to ensure quality of care
• Support quality, compliance, and performance improvement initiatives
