Bridge for Wellness and Therapeutic Counseling, Inc. is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our full‑time team. This role is pivotal in our interdisciplinary chronic pain functional restoration program within a remote hospice center. The LCSW will work collaboratively with a variety of healthcare professionals to deliver comprehensive and empathetic care to patients managing chronic pain and related mental health conditions.

• Conduct thorough assessments of patients’ emotional and behavioral needs through interviews, direct observation, and standardized measurement tools to design individualized treatment plans.
• Utilize a range of therapeutic modalities, including cognitive-behavioral therapy, mindfulness, and supportive therapy, to address issues such as anxiety, depression, grief, and lifestyle adjustments associated with chronic illness.
• Facilitate group therapy sessions and workshops focused on pain management strategies and coping techniques, enhancing patients’ skills for better quality of life.
• Provide expert consultation and emotional support to interdisciplinary team members in a collaborative environment to ensure integrated care that respects psychological perspectives.
• Maintain detailed and accurate medical documentation in compliance with HIPAA and state regulations, ensuring the continuity and quality of patient care.

You will primarily work with individuals suffering from chronic pain conditions, end‑of‑life issues, and associated psychological challenges, often exacerbated by the social isolation that can accompany their diagnoses. Your role is essential in helping these patients navigate their emotional landscapes, improve their coping skills, and restore their quality of life through empathetic and evidence‑based interventions.

• A valid and active LCSW license issued by the State of California.
• A Master's degree in Social Work from an accredited institution.
• A minimum of 2 years of post‑graduate experience in a clinical setting, preferably with a focus on chronic illness or pain management.
• Strong understanding of mental health and psychosocial principles applicable to chronic pain and end‑of‑life care.
• Exceptional communication skills, with the ability to maintain professional boundaries and patient confidentiality in all interactions.

• Experience working collaboratively in an interdisciplinary team, especially within medical or hospice settings.
• Familiarity with telehealth platforms and remote therapy methodologies.
