Vaya Health seeks a TCL Community Liaison to educate members, providers, and community partners about the Transition to Community Living (TCL) program and RSVP processes. You will educate on eligibility, barriers, and access to services, working closely with the TCL team and local hospitals.
You will travel within NC, maintain residency, and operate mostly in the community; assist with referrals, documentation, and quarterly collaborations.
