WorkEnvironment
This position operates primarily in a remote setting, providing the flexibility needed to conduct client assessments and facilitate care plans from the comfort of your own home. However, you will also be required to visit clients at designated facilities or their homes to ensure comprehensive support. Regular interaction with interdisciplinary teams—comprising healthcare professionals, caregivers, and family members—will enhance collaboration and care delivery.
Our commitment to work-life balance means that core hours of operation are Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed.
- Conduct in-depth assessments of seniors' psychosocial needs, accounting for emotional, mental, and social dimensions to create tailored support strategies.
- Develop individualized care plans through collaboration with clients and their families, aligning support with personal goals and preferences.
- Provide counseling services designed to help seniors navigate life transitions, identifying coping strategies and solutions for challenges they encounter.
- Advocate for clients' rights and access to critical community resources, ensuring they receive appropriate services to maintain their independence and dignity.
- Engage regularly with clients through visits to observe their living conditions, monitor progress, and address any emerging concerns.
- Facilitate communication among family members to resolve any conflicts or misunderstandings regarding the care of senior clients.
- Educate seniors and families about available community resources and support groups that foster social engagement and enhance their overall quality of life.
- Maintain accurate and timely documentation of assessments, care plans, and progress notes to ensure compliance with organizational standards and legal requirements.
- Stay informed about current best practices in social work and geriatric care, ensuring adherence to ethical guidelines and regulatory standards.

Qualifications- Bachelor's Degree in Social Work from an accredited institution.
- Current and valid state licensure as a Social Worker.
- A minimum of three years of applicable experience in social work, preferably in a geriatric or community service setting.
- Proficient understanding of social work principles, aging-related issues, and available community resources.
- Exceptional assessment and problem-solving skills, with a focus on client-centered care.
- Ability to build rapport with seniors and families, demonstrating a compassionate approach to care.
- Strong communication skills for effective collaboration with interdisciplinary teams.
- Comfort and proficiency in using electronic health record (EHR) systems and other relevant technology tools.
