Role Responsibilities
- Design and operationalize a human led care coordination service across education, therapeutic, and medical domains.
- Define care coordinator responsibilities, boundaries, escalation paths, and operating principles.
- Build and refine playbooks and SOPs for intake, triage, case planning, referrals, and cross system coordination.
- Establish quality controls and performance metrics for care coordination teams.
- Collaborate cross functionally to ensure scalable, structured service delivery.
- Experience overseeing or operating care coordination services supporting families with complex needs.
- Demonstrated ability to design operating models, scope boundaries, and documentation standards.
- Experience building workflows, playbooks, and quality assurance systems.
- Strong written communication skills and structured operational thinking.
- Ability to work independently in a remote, fast paced environment.
