Location: Northern
CVS Health is seeking a licensed Behavioral Health Case Manager (Bilingual) to deliver telephonic care management for Medicare members from a home office, Monday through Friday business hours.
You will conduct assessments, develop care plans, and coordinate with providers and community resources to improve member well‑being. A Master’s in Social Work or Counseling and a valid state license are required;
Spanish/English fluency is essential for patient engagement and documentation.
