VA Greater Los Angeles Healthcare (VAGLAHS) seeks two RN professionals for Remote Patient Monitoring - Home Telehealth. You will oversee the nursing process, care coordination, patient education, and health promotion, collaborating with the interdisciplinary team.
Responsibilities include RPM-HT care coordination, triage of biometric data, and use of CIS and TMP to guide interventions. Telework not available; schedule Monday–Friday, 8:00am–4:30pm.
