Ennoble Care in Sussex County, NJ is seeking an experienced, full-time Hospice Social Worker to provide compassionate psychosocial support within our home-based hospice program.
You will conduct timely assessments, participate in IDG meetings, document care plans and daily notes, and collaborate with patients, families, Case Managers, and the care team to coordinate quality end-of-life care.
Salary range is $63,000–$70,000 annually, commensurate with experience, with full benefits.
