PharmD Live is seeking a Clinical Pharmacist to deliver pharmacist-led care remotely via telehealth. You’ll conduct chronic care management, transitions of care, RPM, and MTM to improve patient outcomes and value-based success.
Responsibilities include medication reviews, dosing checks, and collaboration with care teams. Minnesota-licensed pharmacists with BCPS/BCACP or willingness to obtain are preferred. Flexible schedule with remote work options.
