This Home Health therapist position is based in Kirkland, Washington, a beautiful lakeside community on the eastern shore of Lake Washington. Kirkland offers a unique blend of scenic waterfront living and urban convenience, with easy access to Seattle, Bellevue, and the greater Eastside. In this role, you will provide home-based physical therapy services to adult and geriatric patients in their residences in Kirkland and surrounding Eastside neighborhoods. The patient population typically includes individuals recovering from hospitalization or surgery, those managing chronic conditions, and older adults striving to maintain independence and safety  will conduct thorough evaluations, develop individualized treatment plans, and deliver skilled interventions focused on functional mobility, strength, balance, pain management, and fall prevention.

A typical day involves traveling between patient homes, performing initial assessments and follow-up treatments, and coordinating care with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, social work, and home health aides. You will perform home safety evaluations, recommend adaptive equipment or environmental modifications, and provide extensive education to patients and caregivers to support long-term success. Patient caseloads and visit volume are structured to allow for quality, patient-centered care and thorough documentation.

You will utilize an electronic medical record system to complete visit documentation, care plans, and communication with the broader care team. Strong organizational skills and clinical judgment are essential, as you will make independent decisions in the field while having access to remote support from clinical leadership. This setting allows you to see the impact of your interventions directly in patients’ daily environments, tailoring therapy to real-life functional goals such as safe transfers, navigation of stairs, and community mobility.

Shifts typically follow daytime hours on weekdays, with scheduling that may offer some flexibility depending on agency needs and patient preferences. Weekend or limited on-call rotations may be part of the role, in alignment with home health standards for continuity of care. As a Home Health therapist in Kirkland, you will be able to practice at the top of your license, build strong relationships with patients and families, and contribute to meaningful rehabilitation outcomes in a community that values wellness and quality of life.
