As a Home Health Physical Therapist with Freedom at Home, you will:

• Evaluate patients in their homes and develop individualized treatment plans

• Provide skilled physical therapy services

• Educate patients and caregivers on exercises, safety, and mobility

• Complete OASIS and clinical documentation in a timely manner

• Collaborate with physicians, nurses, and other healthcare professionals

• Monitor patient progress and adjust treatment plans as needed

• Promote safe, high-quality patient care and excellent outcomes

• Competitive pay: $55 - $65 HOURLY

• Per-Visit (Per Point) Compensation

• Paid mileage

• Graduate of an accredited Physical Therapy program

• Current Michigan Physical Therapist license

• Current CPR certification

• Minimum of 1 year of PT experience

• Home Health experience is a plus but not required

• Valid driver's license, reliable transportation, and auto insurance
