HOW YOU'LL MAKE A DIFFERENCECare and compassion are at the heart of what you’ll do as an Occupational Therapist (OT). You’ll be part of an interdisciplinary team dedicated to providing compassionate, quality care and producing positive outcomes for patients. Provide home‑based therapy as directed by the attending physician, interact with patients and families, and support each step of their recovery journey.

HOW YOU'LL WORKProvide comprehensive skilled services as ordered by the attending physician. Educate patients and their families, ensure patient safety, and accurately document all care while remaining compliant with legal rules and regulations.
MAJOR AREAS OF RESPONSIBILITY- Plan of Care:
Provide skilled occupational therapy as ordered, observe, record, and report patient reactions, changes, and progress to the attending physician and appropriate agency staff. Develop and adjust individualized OT plans as needed. - Evaluation:
Assess the patient’s level of functioning using diagnostic and prognostic procedures. - Patient Outcomes:
Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and activity. - Minimize Patient Risk:
Reduce readmissions by teaching preventive measures and decision‑making skills. - Family Educator/Advocate:
Counsel patients and families on injury/disease management, overall OT phases, and proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices. - Supervision:
Oversee and supervise total patient care provided by a Certified Occupational Therapy Assistant (COTA). - Communication/
Collaboration:
Coordinate with the COTA on supervisory visit results. - Policies:
Complete all clinical documentation according to agency protocol and Medicare/Federal guidelines. - Rules and Regulations:
Understand and follow agency policies, procedures, rules, and regulations; communicate schedule/availability changes to schedulers or supervisors. - Operations:
Attend in‑service training and mandatory agency meetings.
- Compassionate communicator with a positive attitude.
- Patience in working with patients, families, physicians, and coworkers.
- Attention to detail, observant, and diligent in following directions.
- Occupational Therapist licensed in the state of employment.
- Minimum one year of experience in an acute care setting.
- Home care experience preferred.
- Valid driver’s license and auto insurance in your own name.
- Capable of meeting all physical demands of the role.
