• Conduct comprehensive psychosocial assessments through interviews, observations, and review of patient medical records to identify emotional, social, and environmental factors impacting recovery.
• Deliver counseling and emotional support to patients and their families, effectively addressing adjustment issues related to rehabilitation and helping them navigate their feelings during this transition.
• Collaborate with an interdisciplinary rehabilitation team to design and implement individualized treatment plans tailored to the specific psychosocial needs of each patient.
• Facilitate access to vital community resources, such as support groups, vocational rehabilitation services, and housing assistance, ensuring patients have the tools they need for successful rehabilitation.
• Provide ongoing education and resources to patients and families regarding post-rehabilitation services and community options that may enhance their quality of life.
• Engage in discharge planning, assisting with the smooth transition of patients to home or other care settings while ensuring continuity of care.

Your work will significantly impact a diverse patient population encompassing individuals facing a wide range of physical and mental health challenges. The clients you serve may include those recovering from strokes, spinal cord injuries, orthopedic surgeries, neurological disorders, and other life-altering conditions. Your empathetic approach will help patients navigate their physical rehabilitation while addressing the emotional and social adjustments required for successful reintegration into their communities.

• A Master's degree in Social Work from an accredited program is required.
• Possession of an active state license as a Licensed Clinical Social Worker (LCSW) is essential.
• Excellent communication and interpersonal skills are paramount to effectively engage patients and collaborate with medical professionals.

• Prior experience working in rehabilitation or medical settings where you have developed a strong understanding of adjustment issues, coping mechanisms, and grief.
• Experience with telehealth practices is a plus, as it will enhance your capability to deliver patient support in a remote setting.

The annual salary for this position ranges from $75,000 to $100,000, depending on experience and qualifications, along with the opportunity for a sign-on bonus.
