Big Minds, Tiny Hands is seeking a compassionate and motivated Social Worker to provide home-based Early Intervention services to children from birth to age three with developmental delays.
This role may include providing Special Instruction and/or Social Work services , depending on qualifications and program needs. Services are delivered in the child’s natural environment, with a strong focus on supporting developmental growth and empowering families.
This is a fee-for-service position offering flexible scheduling.
Training will be provided for qualified candidates who are new to Early Intervention or Special Instruction services.
- Provide home-based services to children and families
- Deliver Special Instruction and/or Social Work services based on IFSP goals
- Support social-emotional development, behavior, and daily functioning
- Use play-based, routine-based, and family-centered strategies
- Incorporate caregiver coaching to promote skill development in daily routines
- Participate in evaluations and assessments as needed
- Complete daily session notes in compliance with Early Intervention requirements
- Prepare and submit progress reports within required timelines
- Monitor child and family progress and adjust interventions as needed
- Provide guidance to families on behavior management, routines, and development
- Maintain consistent communication with caregivers
- Support families in accessing resources and implementing strategies at home
- Collaborate with service coordinators and multidisciplinary team members
- Follow all New York State Early Intervention Program regulations
- Ensure timely submission of required documentation and forms
- Maintain compliance with service and billing standards
- Master's Degree in Social Work (MSW) or related field
- Active New York State License (LMSW or LCSW) preferred
- Strong communication and organizational skills
- Interest in working with young children and families
- Experience with Early Intervention or pediatric population
- Experience providing Special Instruction services
- Bilingual (Spanish or other languages) is a plus
- Type: Fee-for-Service (1099 or W-2 based on agency structure)
- Setting: Home-based / community-based
- Schedule: Flexible, based on availability
- Caseload: Assigned based on location and availability
- Training: Provided for Special Instruction and Early Intervention services
