This position is remote, allowing you the flexibility to practice from home while effectively managing a caseload of clients. You will work closely with a diverse team of healthcare professionals, including primary care providers and support staff, to coordinate patient care and contribute to a collaborative approach in mental health treatment. Key Responsibilities
  • Provide expert guidance and therapeutic intervention to foster emotional, physical, and mental growth for patients, utilizing evidence-based practices.
  • Conduct comprehensive patient intakes, diagnostic assessments, and functional evaluations to develop tailored treatment plans aligned with patient goals.
  • Engage in brief, focused interventions for patients in need of mental health services, ensuring timely support.
  • Collaborate with primary care providers to identify and manage mental health and psychosocial issues, facilitating referrals to specialty services when necessary.
  • Initiate and maintain follow-up communication with patients to evaluate progress and adapt treatment strategies as needed.
  • Develop relapse prevention plans for at-risk patients to help maintain their well-being and stability.
  • Coordinate the delivery of health services related to behavioral health care, connecting patients with appropriate resources within the community.
  • Maintain accurate clinical documentation and progress notes in compliance with federal guidelines and best practices.
  • Conduct educational sessions to empower patients in self-management and informed participation in their healthcare.
  • Consult with clinic management regarding behavioral health services, contributing to program evaluation and enhancement.
  • Support patients in adhering to medical treatments initiated by primary care providers, offering coping strategies for potential side effects.
  • Adhere to the Code of Ethical Conduct for Clinical Professionals, maintaining the highest standards of practice.
