Job Summary:
The Access to Care Coordinator will lead Covering Wisconsin’s efforts to connect more Wisconsin residents to preventive healthcare providers and services, so that they may live healthier lives. This person will help assess, refine, and deepen existing Covering Wisconsin (CWI) approaches and activities with the aim of developing practices that are most effective and an efficient use of existing internal capacity and external opportunities.
A regular component of the Access to Care Coordinator role will be meeting, virtually or in-person, with individuals and groups around Wisconsin to cultivate relationships and to provide informal and formal presentations about CWI and our health insurance and connection to care services. This position will also work with a partner agency, Center for Patient Partnerships, to enhance coordination and access to care intersection points given their focus on assistance for consumers with chronic conditions.
This is a new position within CWI, funded by a grant award anticipated to continue through October 2030.
The Access to Care Coordinator will be responsible for:
- Identifying access to care barriers; compiling access to care barrier data; monitoring, analyzing, and communicating trends through presentations, informal conversations, and formal reporting; and spearheading exploration of potential educational topics and resources to support CWI efforts and consumer needs.
- Working with other CWI coordinators, managers, and staff to create organization-wide access to care standards and processes. This aspect will also help to discern how best to provide these services, whether by integrating within existing staff roles or pursuing other approaches.
- Fostering communication, coordination, and relationships with external care organizations such as hospitals, health systems, community health centers, and other community health-serving agencies to, in part, support the relevance and accuracy of CWI’s access to care education, services, and approaches.
While Division of Extension headquarters are located within Madison, WI, the position location is flexible and will be determined based on programmatic needs, successful candidate’s preferences, and availability of suitable space.
Partial Remote
This position may require some work to be performed in-person, onsite, at a designated campus work location. Some work may be performed remotely, at an offsite, non-campus work location. Remote work requires an approved remote work agreement (RWA). A RWA requires successful candidates to possess their own high-speed internet and phone to perform the work on a university provided computer. Per University policy, transportation between home and assigned work location is not payable/reimbursable and will be at the expense of the employee.
This position has the possibility of working up to 2 days remote per Extension's Remote Work Policy.
Evening/Weekend Work
This position may be required to work occasional nights and weekends for things such as attending outreach events to meet the program needs and priorities of the communities served.
Travel Expectations
It is expected that employees will make individual arrangements for transportation adequate to meet position responsibilities and essential job functions. Read more about UW-Madison Driver Authorization.
