As a Home Health Pharmacist
, you will review medication regimens, identify potential interactions, and provide consultation to patients, families, and healthcare teams. Your guidance ensures seniors adhere to their prescriptions and experience better health outcomes.

• Evaluate medication profiles, dosages, and potential drug interactions.
• Offer medication counseling, adherence strategies, and patient education.
• Collaborate with physicians, nurses, and other providers to optimize medication therapy.
• Maintain accurate documentation and comply with pharmacy regulations.
• Assist in the development of policies for safe medication management.

• Licensed Pharmacist in California.
• Experience with geriatric patients or home-based pharmacy services preferred.
• Strong communication, analytical, and problem‑solving skills.
• Knowledge of medication management tools and EHR systems.
