Part-Time Weekend Physical Therapist

Role Overview: This position involves providing personalized home-based physical therapy to help patients regain strength, mobility, and independence. The therapist collaborates with healthcare teams, develops and implements treatment plans, assesses patient progress, and documents interventions.

• Plan and deliver prescribed physical therapy treatments in patients' homes to optimize physical function.
• Assess and document patients' physical strengths to guide treatment goals and support physician evaluations.
• Develop rehabilitation programs aligned with physician plans and patient needs.
• Provide clear instructions and oversee performance of PTAs and aides.
• Maintain thorough documentation of patient observations, interventions, and progress.
• Report patient progress to physicians, care teams, patients, and families, ensuring comprehensive communication.

• Current, unrestricted Physical Therapy license.
• CPR certification.
• Experience collaborating within healthcare teams.
• Preferred: one year of physical therapy experience.

Additional Requirements: Valid driver’s license and vehicle insurance are necessary. This is a patient-facing role requiring TB screening and adherence to safety programs.
