Hudson In Home Physical Therapy PLLC delivers outpatient-quality therapy directly in patients' homes, reducing the need for transportation and clinic visits multiple times per week. By providing care in a familiar environment, patients often have more energy for treatment and are more consistent with their therapy sessions. The practice operates like an outpatient clinic but treats patients at home, offering one-on-one, hour-long visits with licensed therapists and no use of aides or assistants.
This individualized approach minimizes distractions and supports better recovery outcomes. For many older adults, in-home therapy also allows clinicians to assess daily living activities and tailor interventions specifically to their home setting.
The Occupational Therapist will provide full-time, on-site care to patients in their homes, based out of Bronx, NY. Responsibilities include evaluating patients' functional abilities, developing individualized treatment plans, and delivering hands-on occupational therapy focused on improving activities of daily living and safety in the home environment. The role involves collaborating with physical therapists and other healthcare professionals, educating patients and caregivers, and documenting clinical findings and progress in a timely, accurate manner.
The Occupational Therapist will also adapt interventions to each patient's goals, monitor outcomes, and adjust treatment plans to support optimal rehabilitation and independence.
- Candidates should possess strong skills in Occupational Therapy and Rehabilitation to design and deliver effective, evidence-based treatment plans.
- Candidates should possess foundational knowledge in Medicine to understand diagnoses, precautions, and interdisciplinary care needs.
- Candidates should possess excellent Communication skills to interact clearly and respectfully with patients, caregivers, and healthcare team members.
- Candidates should possess relevant Pediatrics experience if working with children and adolescents in the home setting.
- Current state licensure (or eligibility) as an Occupational Therapist in New York, and graduation from an accredited occupational therapy program.
- Experience in home-based or outpatient settings, with the ability to work independently, manage time effectively, and travel locally to patient homes.
- Commitment to person-centered care, cultural sensitivity, and maintaining professional and ethical standards.
- CPR certification and familiarity with electronic documentation systems are beneficial.
