Role DescriptionThis full-time, on-site Physical Therapist role is based at Rehab at Shannondell in Audubon, PA. The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver therapeutic interventions to improve mobility, strength, and functional independence. Daily responsibilities include conducting assessments, documenting progress, educating patients and families on exercise programs and safety, and collaborating with physicians, nurses, and other rehabilitation professionals.
The role may also involve coordinating care for patients transitioning between inpatient, outpatient, and home-based rehabilitation settings, and participating in quality improvement initiatives.
- Strong foundation in Medicine and Physical Therapy, with the ability to apply clinical knowledge to diverse patient populations.
- Expertise in Rehabilitation, including designing and implementing individualized treatment plans to restore function and reduce pain.
- Demonstrated commitment to high-quality Patient Care, emphasizing safety, dignity, and clear communication.
- Experience or comfort with Home Care coordination, including discharge planning and patient education for continued progress outside the facility.
- Current state licensure as a Physical Therapist (or eligibility to obtain licensure in Pennsylvania).
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
- Ability to work collaboratively within an interdisciplinary team and manage a varied caseload effectively.
- Solid documentation skills and familiarity with electronic medical records; prior experience in a rehab or senior care setting is beneficial.
