SummaryThe 340B Nurse Coordinator is responsible for coordinating and supporting the organization’s 340B medication program, patient pharmaceutical assistance activities, and medication documentation processes. This position works closely with patients, providers, clinical staff, clinic managers, pharmaceutical assistance programs, and external 340B partners to ensure eligible patients receive needed medications in a timely and accurate manner.
The 340B Nurse Coordinator will oversee the review and documentation of medication-related information, including referrals, specialty consultation notes, medication reconciliation, and prescribed medications associated with 340B referrals. The Coordinator is responsible for ensuring required documentation is complete, accurate, and appropriately recorded in the electronic medical record and 340B reporting processes.
Responsibilities- Coordinate and maintain 340B medication and patient assistance processes.
- Review patient records, referrals, consultation notes, medication lists, and other clinical documentation for completeness and accuracy.
- Ensure required referral and medication documentation is appropriately entered and maintained in the electronic medical record and 340B reporting systems.
- Review and process weekly 340B reports and documentation.
- Coordinate with providers and clinical staff to obtain missing or required documentation.
- Assist uninsured and underinsured patients with pharmaceutical assistance applications and medication access.
- Process, track, and maintain patient assistance applications, medications, and related documentation.
- Document patient, provider, pharmaceutical company, and program communications in the appropriate systems.
- Monitor medication shipments and notify appropriate staff or patients when medications are received.
- Maintain accurate and confidential patient records.
- Assist patients with identifying appropriate community resources when needed.
- Communicate with internal staff, pharmaceutical companies, and external 340B partners regarding medication and documentation needs.
- Maintain compliance with organizational policies, procedures, HIPAA, and applicable 340B requirements.
- Perform other duties as assigned.
- RN or LPN. Must maintain active licensure in the state of Missouri.
- Excellent interpersonal skills as evidenced with staff and patients, to be trusted with confidential information and have the ability to act as liaison between patients and practitioner/staff.
- BLS required.
- Ability to work independently
