At Peace Bridge Hospice, we provide compassionate, end-of-life care that prioritizes dignity, comfort, and emotional support for both patients and families. Our team is dedicated to delivering holistic hospice care and we are seeking a Hospice Social Worker who shares our passion.

The Hospice Social Worker (MSW / LSW / LISW) is a key member of the interdisciplinary hospice team, providing psychosocial support, counseling, and care coordination for patients facing life-limiting illness.

This role focuses on improving quality of life by helping patients and families navigate end-of-life care, grief, and complex healthcare decisions
.

• Conduct comprehensive psychosocial assessments for hospice patients and families

• Provide emotional support, grief counseling, and crisis intervention

• Assist with end-of-life planning, advance directives, and goals-of-care discussions

• Educate families on the hospice philosophy and available support services

• Connect patients to community resources, financial assistance, and social services

• Collaborate closely with nurses, physicians, chaplains, and hospice team members

• Facilitate communication between patient, family, and care team

• Provide bereavement and ongoing family support

• Maintain compliant documentation and participate in care team meetings

• Master’s Degree in Social Work (
  MSW required
  )

• Active Ohio license (
  LSW or LISW required
  )

• Experience in hospice, palliative care, home health, or healthcare setting preferred

• Strong understanding of end-of-life care and grief support

• Compassionate, patient-centered approach

• Ability to work independently in a home-based environment or nursing home environment.

• Valid driver’s license and reliable transportation
