Overview
Physical Therapist Case Manager role, full-time, in-home rehabilitation with Albany Home Health in Albany, OR. Provide in-home physical therapy to help patients regain mobility, improve strength, and maintain independence. Responsibilities- Develop and implement individualized treatment plans.
- Perform patient assessments, including mobility, gait, strength, and range of motion evaluations.
- Provide physical therapy services in accordance with physician-directed care plans.
- Educate patients, families, and caregivers on therapy goals, exercises, and safety measures.
- Monitor patient progress and maintain accurate clinical documentation.
- Collaborate with physicians and interdisciplinary healthcare teams.
- Participate in care conferences and discharge planning.
- Supervise Physical Therapy Assistants and home health aides as required.
- Identify additional patient care needs and coordinate appropriate referrals.
- Current Oregon Physical Therapist license.
- Graduate of an accredited CAPTE-approved Physical Therapy program.
- Minimum 2 years of Physical Therapist experience; home health experience preferred.
- Current CPR certification.
- Excellent communication, organizational, and documentation skills.
- Valid driver's license, reliable transportation, and current auto insurance.
