About This Role

The Care Manager (RN) will support Michigan Medicaid members with complex medical needs through telephonic outreach, health assessments, and care plan development. This role is ideal for those with experience in case management or behavioral health settings.

• Pay: $26.41 - $51.49/hour
• Location: Detroit, MI
• Schedule: Full Time, Day shift (Monday through Friday 8:30AM to 5:00PM EST)

• Complete comprehensive assessments of members per regulated timelines.
• Develop and implement care coordination plans with members and healthcare professionals.
• Conduct telephonic, face-to-face, or home visits as required.
• Perform ongoing monitoring of care plans to evaluate effectiveness.
• Maintain ongoing member caseload for regular outreach and management.
• Promote integration of services for members, including behavioral health and community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivation interviewing to educate and support members.
• Assess for barriers to care and provide coordination assistance.
• May conduct medication reconciliation as needed.

• Education:
  • Registered Nurse (RN) License must be active and unrestricted in state of practice.
• Experience:
  • At least 2 years experience in healthcare, preferably in care management or a medical/behavioral health setting.
