Job Summary:

The Medical Records Clerk is responsible for organizing, managing, and protecting patient health information. This position ensures that medical records are accurate, up-to-date, and compliant with healthcare regulations and privacy laws such as HIPAA.

Filing records and processing release of information requests, receive, scan, and index medical records in the Electronic Health Record/EHR System. They will also scan/fax and send healthcare/medical records to other healthcare facilities upon request.

The HIM Specialist applies strong attention to detail and critical thinking skills to identify documentation deficiencies, support providers in completing records, and ensure compliance with HIPAA and applicable federal and state regulations.

Qualifications:

  • High school diploma or GED required; associate degree in Health Information Management or related field preferred
  • RHIT, RHIA, or equivalent HIM certification preferred
  • Experience with hospital, clinic, or CAH/RHC medical records functions preferred
  • Familiarity with Epic EMR or comparable electronic medical record systems preferred
  • Knowledge of medical terminology, documentation standards, and HIPAA regulations
  • Strong analytical, organizational, and communication skills

Responsibilities include:

  • Review medical records for completeness, accuracy, and compliance with documentation standards
  • Perform chart analysis and deficiency tracking to ensure timely completion of medical records
  • Manage document scanning, indexing, and electronic record maintenance within the EMR
  • Process and fulfill release of information (ROI) requests in accordance with HIPAA and organizational policies
  • Ensure proper patient identification and record integrity across all systems
  • Collaborate with providers and clinical staff to resolve documentation deficiencies and support timely record completion
  • Monitor and maintain compliance with federal, state, and regulatory requirements related to medical records
  • Assist with audits, accreditation readiness, and regulatory surveys
  • Identify trends in documentation deficiencies and support process improvement initiatives
  • Maintain confidentiality and security of all patient health information
  • Document HIM activities and actions within the system as required
  • Maintain established productivity and quality benchmarks
    Stay current with HIM regulations, documentation standards, and system updates

Knowledge, Skills, and Abilities:

  • Knowledge of health information management practices, medical record standards, and regulatory requirements
  • Understanding of HIPAA privacy and security rules
  • Familiarity with clinical documentation standards and medical terminology
  • Strong attention to detail and commitment to data integrity
  • Ability to analyze records and identify discrepancies or deficiencies
  • Effective written and verbal communication skills
  • Ability to prioritize workload and meet productivity standards
  • Proficiency with EMR systems (Epic preferred) and document management tools
  • Ability to work independently and collaboratively across teams

Physical Requirements for the Role:

  • Prolonged sitting and computer use
  • Manual dexterity to operate a keyboard and standard office equipment
  • Ability to review detailed electronic medical records for extended periods
  • Protentional for Hybrid office-based and remote work environment
  • Limited direct patient interaction; frequent collaboration with providers and clinical staff
  • Standard work hours with overtime as needed
