Remedy Home Health Care is actively seeking a licensed Physical Therapist (PT) to join our growing Home Health team in Orange County
.
This Per Diem Physical Therapist role offers flexibility, autonomy, and the opportunity to provide high-quality rehab and community-based care directly to patients in their homes.
As a Home Health Physical Therapist
, you will work with senior care populations
, including geriatric patients recovering from illness, injury, or surgery. You’ll deliver skilled therapy services in home-based, assisted living, and community care settings
, helping patients regain independence and improve their quality of life.

Home Health Physical Therapist (PT)
As a Physical Therapist in Home Health
, you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients
.
- Conduct in-home physical therapy evaluations and reassessments
- Develop and implement personalized rehabilitation care plans
- Improve strength, balance, mobility, and functional outcomes
- Provide therapy in home health, assisted living, and community care settings
- Educate patients and families on fall prevention and safe mobility techniques
- Document visits using Well Sky (Kinnser) EMR
- Collaborate with interdisciplinary home health teams
- Licensed Physical Therapist (PT) – California (Required)
- CPR Certification (AHA approved)
- Minimum 1 year of clinical experience (Home Health preferred)
- Experience in senior care, geriatrics, rehab, or community-based care preferred
- Strong clinical, communication, and patient education skills
- Valid driver’s license, reliable transportation, and auto insurance
- Flexible Per Diem / PRN
- Competitive per-visit pay + mileage reimbursement
