Homeside Rehab is seeking compassionate and motivated Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to provide one-on-one home-based therapy services in Sundown, NY 12740
.

• Provide skilled Physical Therapy services in the patient's home
• Conduct evaluations and develop individualized plans of care, as applicable to your professional scope of practice
• Carry out established treatment plans and therapeutic interventions
• Provide therapeutic exercises, strengthening, balance training, gait training, mobility activities, and other interventions within your professional scope of practice
• Monitor patient progress and communicate changes or concerns to the appropriate supervising therapist and clinical team
• Educate patients and caregivers on safety, mobility, home exercise programs, and functional independence
• Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements
• Communicate professionally with patients, caregivers, therapists, and office staff
• Provide one-on-one therapy services in the patient's home

• Active New York State Physical Therapist (PT) or Physical Therapist Assistant (PTA) license

• Basic computer proficiency for electronic documentation
• Strong interpersonal and communication skills
• Ability to work independently while following established plans of care and supervision requirements
• Valid driver's license and reliable transportation for home visits
• Home health, outpatient, rehabilitation, or skilled nursing experience preferred
• Minimum of 1 year of clinical experience preferred

Job Type: Full-Time, Part-Time, Per Diem

Pay: $45–$100 per hour
, based on discipline, experience, and case assignment
Schedule: Flexible – choose your availability
Shifts: Day and evening availability, Monday through Friday, with optional weekend availability
