- Work with the nursing team to ensure all medical documentation is current for incoming participants.
- Provide quality customer service to all parents, staff, funders, and participants with regular and consistent communication.
- Tracks and completes all necessary documentation for participants being referred to the Respite program.
- Updates the attendance calendar weekly and schedules staff accordingly.
- Reviews monthly reports and safety drills submitted by staff for accuracy, acknowledges documentation prior to submission.
- Provide physical assistance including lifting to support participants in accordance with activities involving personal hygiene, activities of daily living, and other domestic functions and in accordance with their support plan. Assist individuals who may weigh more than 150+ pounds using proper transferring techniques and/or mechanical lifts.
- Ensuring the Health and Safety of clients and client rights remains a priority. Supports clients with establishing self-determination, advocacy, and security.
- Provide home-based services in accordance with the funder and agency’s policies and maintain compliance with all the program’s regulatory codes and requirements (i.e., Medicaid, DMH, SB40 Boards, CARF, Columbus Group).
- Managing and supervision of employees: coaching employees, planning, assigning, and delegating work; appraising performance; rewarding and coaching employees; addressing complaints and resolving problems.
- Support peers and supervisors in a collaborative effort to ensure client’s needs are met.
- Complete all required reporting accurately, in a timely manner and in accordance with guidelines established by the Agency’s EHR policy and other Regulatory Entities including but not limited to service documentation, medication administration, incident reports and activity records.
- Maintain HIPAA compliance and confidentiality.
- Demonstrates excellent defensive driving skills, able to operate agency vehicles varying in size from a standard van to a large ADA custom van and a driving record that meets agency policy requirements.
- Safeguard and assure proper care and maintenance of clients and Agency property including but not limited to adaptive equipment, lifts, and Agency vehicles.
- Effectively communicate orally and in writing. Respond to written communications before the next business day.
- Complete weekly purchase of supplies including: groceries, hygiene items, cleaning products, activity items.
- Adequately assess and respond to emergencies at any time apart from scheduled PTO.
- Create and execute activities and community outreach opportunities.
- Comprehension and proficiency with all agency software used for programs.
- Secure and maintain training/certification in CPR/First Aid, CPI, L1MA, abuse and neglect, blood borne pathogens, HIPAA, and all other certifications or trainings as required by the Agency.
