Diabetes Educator

Full-time, 3.5-year, grant-funded position. The Diabetes Educator will lead weekly group education classes, delivering both diabetes disease state education as well as financial education from an established curriculum. The Diabetes Educator will provide intensive health, wellness and financial coaching as well as disease state management for study participants. Coaching visits take place monthly and may occur in-person or remotely, depending on the participant's preferences.

Comprehensive documentation will be required. Lastly, the Diabetes Educator may assist with other study-related responsibilities including administering the food pharmacy benefit, participant recruitment, screening for eligibility to enroll in the study, data collection and data entry. The position is hybrid with up to three days per week remote work, one in-person evening per week for group classes, no holidays or weekends.

  • 70% Coaching visits (providing disease state management, health and wellness coaching and financial coaching, visit documentation).
  • 15% Group education classes
  • 10% Food pharmacy administration
  • 5% Recruitment, data collection, data entry

Qualifications:

  • Health Care Professional (PharmD, RN, NP, PA, APRN, RD) licensed in the state of Nebraska. Certified Diabetes Care and Education Specialist credential, preferred. Experience with health and wellness coaching, diabetes education and/or disease state management, preferred. Experience working with low-income populations, preferred.

Knowledge, Skills, and Abilities:

  • Ideal candidate would have excellent oral and written communication skills including the ability to speak in public, counsel patients, motivate patients toward positive behavior change, document patient visits, and communicate via email with research team and study participants.
  • Ideal candidate would be knowledgeable in diabetes pathophysiology and treatment (both lifestyle and pharmacotherapy). Candidate will receive on the job training in financial literacy and coaching.
  • Ideal candidate works well with underserved populations, demonstrates empathy, and has a passion for social justice.

Licenses/Certifications:

  • Nebraska state licensed Pharmacist, Nurse, Dietician, or other health care professional. Certified Diabetes Care and Education Specialist (CDCES), preferred.
