Certified Peer Support Worker- Santa Fe, NM

Candidate must live in Santa Fe, NM or surrounding area. Work at home position with extensive travel in the community.

• Provide non-clinical, evidence-based peer support services; act as a member advocate and share coping skills.
• Assist members in regaining control over their lives through individual recovery planning, wellness self-management, and strengthening resilience to improve personal health outcomes.
• Role model competency in recovery, resilience, and wellness practices.
• Provide peer support to members with mental health and substance use conditions requiring intensive wellness support and recovery-oriented interventions.
• Guide members in creating Wellness Recovery Action Plans (WRAP®) to identify wellness self-management and crisis prevention strategies based on member choice.
• Utilize the 8 dimensions of wellness to help members identify social determinants of health needs and determine whole health goals and objectives.
• Guide members in identifying options and participating in all decisions related to establishing and achieving recovery goals, functioning as a member of the treatment/recovery support team.
• Guide members in developing problem-solving skills to respond to challenges in the recovery process.
• Implement strategies to help members keep important follow-up appointments by identifying barriers and collaborating to find solutions.
• Engage members in inpatient settings to facilitate post-crisis planning, assist with transitions from treatment settings, and navigate multiple systems.
• Facilitate community participation and inclusion for members, encouraging exploration and pursuit of community roles.
• Perform ongoing interventions to engage members in traditional and nontraditional health services and supports, and community and social support networks, including peer, parent, and family support services.
• Strategically share lived experience to inspire hope, empowerment, and positive action.
• Implement, coordinate, and monitor strategies for members and families to improve health and quality of life outcomes.
• Advocate for member needs, identify and address gaps in services and supports.
• Educate providers, staff, members, and families on resiliency and recovery principles, strategies, and tools.
• Facilitate a team approach to member care, including with the Magellan care coordination team.
• Document all interactions according to company standards.
• Maintain professional responsibility, respond appropriately to personal stressors that impact job performance, and recognize crisis situations or risks to member safety and respond appropriately.
• Travel to meet members within the community.
• Perform other duties as assigned.

• 2+ years experience providing peer support to children or adolescents who have received services from a child-serving agency for mental health or co-occurring related issues.
• CPRS/CPS/CPSS/CRPS/PRSS, Peer Specialist certification as required by state.
• Must obtain the National Peer Support Credential (NCPS/NCPRSS) within one year of hire.
• Must be a self-identified person in recovery who is a current or former user of mental health/substance use services.
• Must have a vehicle in good, working condition with the ability to travel within the field regularly.
• Working knowledge of Microsoft Office Product Suite.
• Ability to make decisions requiring significant analysis, quick original and independent thinking.
• Ability to determine appropriate courses of action in complex situations not addressed by existing policies or protocols.
• Knowledge of local behavioral health and community systems, wellness strategies, resilience and recovery principles, practices, and tools such as WRAP, wraparound process, and community-based peer, family, and parent support organizations and services.
• Strong interpersonal and organizational skills and effective verbal and written communication skills.
• Ability to represent strengths and needs of members in clinical settings.
• Ability to summarize and document findings and maintain complete and accurate records.
• Must be able to work effectively, independently and in a team, and prioritize in a fast-paced environment to meet the demands of the organization.
