The Care Manager – Behavioral Health role is a primarily telephonic care manager based in Oklahoma. The individual will assess the needs of enrollees, create care plans, and coordinate services to achieve optimal wellness. The role is part of Humana Healthy Horizons, focusing on helping people achieve their best health.

• Perform telephonic and face‑to‑face assessments and evaluations to guide members/families toward appropriate resources for overall wellbeing.
• Ensure members are progressing toward desired outcomes by continuously monitoring care through assessments.
• Create member care plans and align them with department and organizational objectives.
• Apply a variety of strategies and techniques to manage behavioral, physical, environmental, and psycho‑social health needs.
• Identify and resolve barriers that impede effective care and ensure continuous monitoring of assessments.
• Make decisions about work methods in ambiguous situations with minimal direction.
• Follow established guidelines and procedures.
• Collaborate with providers and community services to promote quality and cost‑effective outcomes.

• Reside in Oklahoma.
• Active RN license or LCSW, LPC, or LMFT license.
• Minimum 1 year of behavioral health clinical experience.
• Minimum 2 years of in‑home case or care management experience.
• Experience coordinating services for Medicaid and/or Medicare enrollees.
• Exceptional communication and interpersonal skills, able to quickly build rapport.
• Knowledge of community health and social service agencies and other community resources.
• Comprehensive knowledge of Microsoft Office applications (Word, Excel, PowerPoint).
• Periodic travel to Humana Oklahoma City office for meetings and training.

• Case Management Certification (CCM).

Travel – Requires a valid driver’s license and reliable transportation; occasional travel within the state may be required.

Work From Home – Eligible employees must provide internet service meeting minimum download speed of 25

Mbps and upload speed of 10

Mbps; a dedicated workspace free from interruptions is required.

Scheduled weekly hours: 40

Pay range: $59,300–$80,900 per year (may vary based on experience and location).

Eligible for a bonus incentive plan based on company or individual performance.
