Job Description

Summary:

The Registered Nurse (RN) is responsible for planning,organizing, and directing home care services while directly reporting to the Clinical Supervisor(s). They possess extensive nursing experience with a focuson community health education and practice. The RN utilizes community resources to develop and implement services tailored to meet the needs of individuals and families within their homes and communities.

Job Duties (include, but not limited to):

• Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs in their homes, including pain assessment and management, medication reconciliation, and evaluation of their support systems.
• Develop individualized care plans in collaboration with the interdisciplinary team, including physicians, social workers, and therapists, to address patients' goals, preferences, and needs.
• Provide skilled nursing care and interventions based on assessment findings and physician orders, including wound care, catheter care, infusion therapy,and palliative care measures.
