** Sentara Norfolk General Hospital has a full-time APP to join Sentara's
** Case Management Pharmacy Program, VA**. Work from home or at the hospital! ** Position Highlights:*
* · Join an experienced team of 1 MD, 9 nurse case managers, 5 clinical pharmacists, and 1 nurse supervisor

Schedule:

Mon – Fri, 8:30AM – 5:00PM, no weekends or holidays and working from home is available
· Duties:
Perform health risk assessments, re-write prescriptions
· Review 150 patients per month
· Work with Sentara Medicaid only
· 1 year experience required as APP in Family Medicine
· Experience in Rheumatology or Infectious Disease preferred

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $ - $ annually.

Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.

Note:

If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
