Make a meaningful difference while providing personalized, one-on-one care. Elara Caring is seeking a Physical Therapist to serve patients throughout the Danbury, Connecticut territory. In this field-based role, you will help patients regain strength, mobility, and independence in the place they feel most comfortable: home. You will have the independence to manage your patient visits while receiving support from an experienced clinical team.

• Evaluate patients’ mobility, strength, safety, and functional needs
• Develop and implement individualized treatment plans
• Help patients improve mobility and safely perform daily activities
• Educate patients and caregivers on exercises, equipment, and safety techniques
• Monitor patient progress and adjust treatment plans as needed
• Communicate changes in condition to physicians and members of the care team
• Complete accurate and timely clinical documentation
• Collaborate with nurses, physicians, therapy professionals, and other team members
• Support patients in reaching their highest possible level of independence

• Graduate of an American Physical Therapy Association-approved physical therapy program
• Active and unrestricted Connecticut Physical Therapist license
• At least one year of experience in a healthcare setting
• Strong communication, organization, and patient-care skills
• Ability to work independently in a home-based environment
• Willingness to travel throughout the assigned Danbury territory
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50%
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Home health experience is valuable, but candidates with transferable clinical experience are encouraged to apply.
