RN Field Clinical Care Coordinator - Plymouth County, MA

About This Role

The RN Clinical Care Coordinator will be the primary care manager for a panel of members with complex medical and behavioral needs, focusing on care coordination to promote appropriate utilization of services and improved quality of care.

• Location: Bridgewater, MA
• Schedule: Full Time, Day shift
• Residency: Must reside in Plymouth County, MA and surrounding areas
• Travel: Local travel up to 75%
• Work Style: Field-based position with a home-based office
• Pay: $29 - $52 per hour

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans addressing chronic health conditions and medication management.
• Partner and collaborate with internal care teams, providers, and community resources to implement care plans.
• Provide education and coaching to support member self-management of care needs.
• Support proactive discharge planning and manage care transitions following ER visits or admissions.
• Advocate for members and families to ensure their needs are represented by the healthcare team.

Education:

• High School Diploma/GED required

Experience:

• 2+ years of clinical experience required

Licenses &

Certifications:

• Current and unrestricted independent licensure as a Registered Nurse for MA required
