AEIC Case Management is seeking certified Community Health Workers to serve as Case Managers in the CARES program, focusing on Medicaid‑compliant care coordination, resource navigation, client education, and advocacy for children and pregnant women.
This is primarily a remote role, but candidates must reside in the Dallas–Fort Worth area, know local resources, and be available for occasional community activities. Excellent HIPAA and confidentiality practices are essential.
