AEIC Case Management LLC is hiring Texas DSHS-certified Community Health Workers (CHWs) as 1099 Independent Contractors under the Bridgewell program to provide resource navigation and care coordination for Medicare clients in Dallas/Fort Worth and surrounding areas.
CHWs will assist with access to medical, behavioral health, housing, transportation, food, and wellness resources, fostering empowerment and improved health outcomes. Requires CHW certification and 1 year of relevant experience.
